Understanding the Influence of Transformational Leadership on Organizational Culture in Healthcare Institutions

This study aimed to explore how transformational leadership influences organizational culture in healthcare institutions through the lived experiences of healthcare professionals. This qualitative study employed a thematic analysis approach based on semi-structured interviews with 16 healthcare professionals from various hospitals and clinics in Tehran. Participants were selected through purposive sampling to ensure direct experience with transformational leadership. Data collection continued until theoretical saturation was achieved. Interviews were audio-recorded, transcribed verbatim, and analyzed using NVivo software. Thematic coding followed Braun and Clarke’s six-step framework, integrating both inductive and deductive strategies to capture nuanced insights into leadership and cultural dynamics. The analysis revealed three major themes: (1) visionary leadership practices, (2) cultural transformation processes, and (3) outcomes of transformational leadership. Visionary leadership practices included inspirational motivation, intellectual stimulation, individualized consideration, and strategic communication. Cultural transformation was facilitated through value internalization, team cohesion, and learning orientation, while overcoming resistance to change. Outcomes reported by participants included enhanced staff empowerment, stronger organizational commitment, improved communication culture, patient-centered care practices, and heightened accountability. Participant narratives highlighted that transformational leaders influenced organizational norms not only through formal strategies but also via symbolic actions and ethical modeling. Transformational leadership plays a critical role in shaping and sustaining a constructive organizational culture in healthcare settings. By inspiring staff, fostering collaboration, and reinforcing shared values, such leadership contributes to a more empowered, communicative, and patient-centered work environment. These findings underscore the need for healthcare systems to invest in leadership development programs that cultivate transformational competencies among current and emerging leaders.
